TheFill.In Provider Terms of Use

Welcome to TheFill.In. By accessing or using our services as a dental staffing agency, you agree to comply with and be bound by the following terms and conditions (“Terms of Use”). Please read these Terms carefully before using our services.

1. Agreement to Terms
   By registering as a provider with The Fill In, you agree to pay the following rates for staffing services:
   • Dental Hygienist: $100 per day
   • Dentist: $200 per day
   • Dental Assistant: $60 per day
   • Front Office: $60 per day
2. Provider Responsibilities
   As a provider, you acknowledge that:
   • The Fill In will thoroughly vet all applicants to ensure their qualifications and professionalism.
   • All dental offices utilizing our staffing services have been properly vetted, and we ensure that all necessary licenses are current and valid.
3. Payment Terms
   Payments for dental professionals will be made directly from the hiring dental office at the conclusion of each completed shift.
   For shifts that extend beyond one week, payments must be made on a weekly or bi-weekly basis, adhering to the same structure as in-office employees.  Dental Professionals will receive W-2/1099 forms from the HIRING Dental Practice for tax purposes, reflecting their earnings.
4. Responsibilities of The Fill In
   The Fill In is dedicated to maintaining the quality and compliance of our staffing services. However, we assume no responsibility for the conduct of dental professionals, payments made by dental offices, or any other liabilities arising from the provision of our services.
5. Insurance and Licensure
   As a provider, you acknowledge that:
   • You are responsible for your own workman’s compensation, licenses, and insurance as required by your state and local regulations.
   • You are working under your own office’s policies, procedures, insurance, and a safe and compliant environment. Upon both practices and professionals signing up with The Fill In, their license has been checked and verified. Going forward, it will be on the practice to confirm that their temporary employee is still working with a valid license.
6. Limitation of Liability
   The Fill In will not be liable for any indirect, incidental, or consequential damages arising from your use of our services. By agreeing to these Terms of Use, you waive any claims against The Fill In related to your use of the staffing services.
7. Modifications to Terms
   The Fill In reserves the right to modify these Terms of Use at any time. Any changes will be effective immediately upon posting on our website. Continued use of our services after any modifications signifies your acceptance of the new terms.
8.  Governing Law
   These Terms of Use shall be governed by and construed in accordance with the laws of [Insert State]. Any disputes arising from these terms shall be resolved in the appropriate courts of [Insert State].
